On 11/18/2022 at 8:15am, Licensing Program Analyst (LPA) Christina Uribe conducted an Unannounced Annual Required Inspection. LPA met with director, Sue Silliman, also present at the time of the inspection were 14 staff & 79 children. The facility is within ratio & capacity compliance today. LPA provided facility representative the Entrance Checklist (LIC 125). The facility was toured to conduct a Health and Safety inspection. The facility currently operates 7:30am-5:30pm, Monday-Friday in 6 classrooms. The classrooms are the Jumping Beans (C2), Lovable Ladybugs (C3), Fabulous Frogs (C4), Playful Puppies (C5), Bouncing Bears (C6), & Loving Lambs (C7). The facility does have a license for a Toddler Component, however they have not enrolled any children under 2 years of age within the past few years.

Classrooms: All classrooms were inspected for age-appropriate furnishings, equipment, & adequate storage for children’s belongings. LPA observed the cleanliness of floors & surfaces, the presence of a fully functional carbon monoxide detector, smoke detector/fire alarms, and a fully charged 3A40BC fire extinguisher that is accessible throughout the facility. The center is equipped with a fully stocked first-aid kit and available in the classrooms.

Food Service Areas: All center provided food items are properly labeled & stored separately from cleaning supplies. Food prep area is clean, adequately equipped, & free from hazardous materials. Snack menu is posted one week in advance, available for review, & dated.

Bathrooms: Facility has separate staff and child designated bathrooms. Toilets and faucets are in safe and sanitary operating condition. The children are able to reach the sinks and toilets and supplies are available. All generic spray bottles used are properly labeled with the contents of the cleaning products inside.

Outdoor Play Area: There are no bodies of water, or free-standing water accessible to children. There are age appropriate toys and materials for the children. The playground outside is fenced and all equipment and surfaces are free from hazards.

Records: LPA reviewed 6 children’s files and 5 staff files. LPA reviewed the facility roster & personnel record and a copy of both was obtained. At least one opening/closing staff member has a current Pediatric CPR/First-Aid Certification. Mandated Reporter certificates were reviewed. Director’s CPR/First-Aid Certificate is current and expires on 04/29/23. The center is in compliance with sign in and out procedure. Emergency Drills are recorded and performed at least every six months. Per facility representative, there are no firearms on the premises. All required documents are posted in a publicly accessible area.

Health-Related Services: This facility does provide Incidental Medical Services (IMS). LPA inspected storage of medications and equipment/supplies, and reviewed children’s, personnel, and administrative records. Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Effective August 1, 2003 California Law requires Child Care Licensees to report unusual incidents or injuries to children in care to child’s parents and to the Department of Social Services using the Unusual Incident/Injury Form (LIC 624). Incidents must be reported within 24 hours to the regional office by phone and the written report, LIC 624, within 7 business days.

Lead Testing Requirement: LPA Uribe and director discussed the upcoming deadline of January 2023 for all child care centers requiring to perform a lead test of all faucets within the facility. Director stated that the facility is already in process of completing this requirement before the deadline.

Deficiencies & Advisory Notes issued during today's inspection: Please see attached deficiency & advisory note pages for additional information

  • Type A Violation: One staff member's criminal background check is still in process and therefore does not have an eligible clearance.
  • Type B Violation: Items that need to be stored in a manner which is not within reach of children were found to be in low, unlocked cabinets or drawers. These items included toileting wipes, disinfecting wipes, shaving cream and adult scissors.
  • Technical Violation: All solid waste bins need to be equipped with a tight fitting lid.
  • Technical Violation: All medications must be kept with their original packaging and prescription label.

LPA Uribe informed the director, Sue Silliman, that this report dated 11/18/2022 documents one Type A citation which shall be posted for 30 consecutive days as there is/are immediate risk to the health, safety, or personal rights of children in care.

Also, LPA Uribe informed the director to provide a copy of this licensing report dated 11/18/2022 that documents any Type A citation to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. Exit interview conducted and report was reviewed with the director, Sue Silliman.
