AMENDED REPORT
On May 29,2026, Licensing Program Analyst (LPA) Tasha Alexander conducted a subsequent inspection to deliver the findings to the above complaint allegation. LPA met with Program Director David Rodgriguez and Administraive director Alexandra Hilaro and explained the purpose of the visit.

During investigation,LPA conducted record reviews, interviews and observations. LPA reviewed video record footage obtained by the facility that shows a child was left unattended in a bathroom for 49 seconds. LPA observed a preschool class along with 3 staff. Two of the staff member are observed transitioning the class to lunch, leaving the 3rd to supervise the bathroom. The 3rd staff, is then observed escorting one child from the bathroom to lunch. A second child is then observed on the video footage, to emerge to the threshold of the bathroom. There is no adult supervision. After 49 seconds, another staff member is observed walking by the bathroom and notices the child. The staff member escorted the child to join his/her class. This poses a postential health & safety risk to children in care.
Based on LPAs observations and interviews which were conducted and record reviews, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED.
California Code of Regulations, Title 22, Division 12, Chapter 1,101229 are being cited on the attached LIC. 9099D.

An exit interview was conducted with program director David Rodgriguez.
A notice of site visit was given and must remain posted for 30 days.
