On 4/15/2026 Licensing Program Analyst (LPA) Tasha Alexander met with assistant director Cynthia Andrews for a subsequent visit to deliver the findings to the above complaint allegation.

The complaint statement alleges staff did not prevent child from engaging in a physical altercation with another child in care. LPA has conducted facility visits which included records reviews, and staff interviews. During the investigation it was confirmed that two children engaged in a physical altercation on the facility's playground which resulted in an injury to a child. It was also discovered that the altercation was not observed by staff members which posed a health and safety risk to children in care.

Based on LPAs interviews and record reviews which were conducted, the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12 & Chapter 13 are being cited on the attached LIC. 9099D.

Today a copy of the LIC 9224 Acknowledgement of Receipt of licensing report was gven to assistant director Cynthia Andrews. This report with 1 Type A citation shall be posted for 30 consecutive days. LPA also instructed Ms. Andrews to provide a copy of this report to parents/guardians that this currently enrolled by the next business day, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224) must be placed in the child's file for verification.

Exit interview was with Assistant director Cynthia Andrews. A Notice of Site Visit was given and must remain posted for 30 days.

(3) To be free from corporal or unusual punishment, infliction of pain, humiliation, intimidation, ridicule, coercion, threat, mental abuse or other actions of a punitive nature including but not limited to: interference with functions of daily living including eating, sleeping or toileting; or withholding of shelter, clothing, medication or aids to physical functioning.

The facility has held a 3 day all staff training supervision and personal rights of children in care, which focused on active supervison of children in care and keeping them safe. Licensee will submit documents summarizing the training to community care licensing along with a written plan on how the facility will no supervise the children and keep them safe while in care. documents must be submitted by 4/23/26.

This requirement was not met as evidenced by record reviews and interviews which revealed staff did not prevent child from engaging in a physical altercation with another child in care. This poses an immediate threat to the health and safety and personal rights of children in care.

During the investigation it was confirmed that staff failed to provide proper supervision to children in care, which resulted in children engagining in an altercation.

101229 Responsibility for Providing Care and Supervision
(a) The licensee shall provide care and supervision as necessary to meet the children's needs.
(1) No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation.

Licensee will submit a written plan on how and who will supervise and shadow the child to keep children in care safe at all times. The written plan must submitted to community care licensing by 4/23/2026. The assistant director was also notified that an informal conference my be required by Community Care licensing.
