25 | On 10/17/2022 at 9:07am Licensing Program Analyst (LPA) Morgan Pringle met with Director Jamie Czelusniak, for an Unannounced Annual Inspection. During the inspection there were eighteen (18) infants and seven (7) staff present. Two classrooms, Infants and Toddlers, were toured for a health and safety inspection. The facility operates from 6:30am – 6:00pm, Monday – Friday. This facility holds licenses for preschool (Facility #013412608) and school age (Facility #013412609) components as well.
The facility has ample age appropriate materials in the classrooms that were observed to be clean and in good condition. All toxins, cleaning products, and hazardous materials were observed to be in inaccessible areas. All napping equipment is properly stored, clean, and free from defects. All cribs are clean and free from hazards. There are three (3) emergency cribs in the Infant room. Both changing stations are properly padded, clean and free of any defects. All children have access to clean drinking water in and outside of the classrooms. There are carbon monoxide detectors, smoke detectors and fully charged fire extinguishers on site. The outside play yard is fully fenced, clean, and free from hazards with plenty of shade. The play structure is sturdy and anchored into the ground. LPA did not observe any harmful or unattended bodies of water in or around the facility.
The facility is operating within its licensed capacity and is in ratio. All proper postings are made visible in the entry way of the facility. All food provided by the facility is properly stored and labeled. All food brought from home is properly stored and labeled as well. The kitchen and food preparation areas are clean and free from all hazards. The fire/disaster drill log was is complete with the last drill logged on 9/27/2022. LPA obtained the sign in/out log. Four children were not properly signed into the facility by their authorized representative (See LIC809D). All staff have obtained a criminal record clearance. LPA obtained the facilities files, children’s files and staff files. Through record review it was found that three (3) infants, who were not present during the inspection, were missing a Needs and Services Plan (See LIC9102-TV). Continued on LIC809-C |
