On 1/30/2024 at 10:05am, a Prelicensing inspection was conducted by Licensing Program Analysts (LPAs) Melanie Otsuji and Simerjit Kaur and Licensing Program Manager (LPM) Wynn Norona. LPA met with Applicant, Coreen Hudson and Director, Sarah Baker. The applicant has submitted an application for a PRESCHOOL license for 45 preschoolers. Also present during the inspection were 3 staff members and 16 preschool aged children. The program will operate from 7:00AM - 6:00PM, Monday through Friday, in 4 classrooms (Room 3, Room 7 (AM USE ONLY), Room 8 and Room 9. A health and safety inspection was conducted inside and outside. Measurements were taken and are as follows:

INDOORS: 2589.31 square feet = 73 children
OUTDOORS: 4501.12 square feet = 60 children

The center has obtained an approved fire inspection clearance from the Livermore Fire Department on 1/12/2024.

The preschool rooms are equipped with varied age appropriate materials and equipment. There are 3 toilets, and 5 sinks available for children use. Staff toilet is located within the main office. Children bring lunch from home and facility will provide AM/PM snacks. Menu will be posted at least one week in advance in a place visible for the authorized representative to see. Facility will utilize office and staff bathroom as isolation space. Cots and mats were observed today.
Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for
drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test.
For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP).

This facility plans to provide Incidental Medical Services - IMS. The play yard is fenced in all around. On the play yard trees are observed to give shade, however during the winter trees are bare. Facility was advised to obtain additional shade during the winter months. The center utilizes a water service to provide clean drinking water inside and outside, with children bringing their water bottles from home. No cleaning agents were found stored with food. There is a refrigerator available. First aid supplies are available in the center. There is a working telephone on site. Facility utilizes a paper sign in/out sheet which has enough room to record the day/time and full legal signature of the authorized representative. LPA reviewed with Applicant the LIC 311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted. Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment. Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.

Director was reminded that California Law requires all facilities to report unusual incidents or injuries to children in care, to child's parents, and to the Department of Social Services using the Unusual Incident/Injury form (LIC 624). LPA Otsuji informed Director that Unusual Incidents include any construction, structural changes, location changes and/or damages to the facility. Incidents must be reported within 24 hours by phone, fax, or email.

Once license is granted, facility will be issued a probationary license for two years and facility agrees to follow the terms of the stipulation and waiver and order issued to facility. Probationary license will be granted once the following items are received:

- Current LIC 309 (Administrative Organization)
- LPAs and LPM observed the blacktop to be lifting causing a potential tripping hazard. Facility plan to prevent tripping hazard to be submitted.
