On September 21, 2022 at 9:00AM Licensing Program Analyst (LPA) Kelly Phan arrived for an unannounced Required-1 Year inspection, and met with center director, Yvonne Ma. There were 55 preschool aged children and 10 fingerprint cleared staff members present during the inspection. The facility is in ratio today. The facility was toured for a health and safety inspection. Hours of operation are Monday through Friday, 7am to 6:00pm.

CLASSROOMS: The six (6) classrooms were inspected; there is one classroom that is not being use right now due to low enrollment. There are adequate play and learning materials available. The floors, furniture, and equipment are age appropriate and in good repair. There is adequate heating/air conditioning, ventilation and lighting. Drinking water is available inside and outside of the center; water fountains are not being used due to COVID. There is proper individual storage space for each child. The isolation area for sick children is the office area, away from other children in care. The center has a maintenance person who checks smoke detectors, carbon monoxides, working telephone, and 2A10BC fire extinguishers throughout the building every month. LPA observed that the facility had new layouts; Director was made aware that any new changes to the facility should be notify to Licensing; per director, she had put in "work order" but was not sure when the new development was going to be executed until the staff arrived at the facility.

BATHROOMS AND TOILETING AREAS: The staff's bathroom is separate from the children's bathroom. All sinks and faucets are in safe and sanitary operating condition. The children can reach the sinks and toilets. Paper supplies are available to the children. There were no hazardous items present today.

FOOD SERVICE AREAS: This facility provide snacks and lunches for the children. There are weekly menus posted at the facility and is available through electronic application for parents and is changed every quarter. There were no chemicals or hazardous items present; All food containers are labeled and have dates on it.

OUTDOOR PLAY AREAS: There are multiple age appropriate play equipment available that is in good repair. There's also a canopy and trees that provides shade to children while at play. Per center director, they would occasionally have water or sand play but is changed periodically to prevent any risks to children.

RECORDS: Five (5) children's files and five (5) staff files were reviewed. There were 4 out of 5 staff files that did not have current mandated reporter training available, see LIC 809D; all children files were complete. LPA reviewed the facility roster and obtained a copy. Director has CPR and First Aid certificate valid until 12/05/2023. The center is in compliance with the sign in and out procedure. Fire and earthquake drills are being conducted at least once every 6 months and the last one conducted was on 08/3/2022 and 06/3/2022. All required documents are posted in a public accessible area.

HEALTH RELATED SERVICES: There's no IMS being provided at this time and no medication being stored or given at the facility. Center director is aware to obtain proper documentation for any child who needs medical services. The center is equipped with a first aid kit that is available in each classroom.

There is one deficiency cited today. Type B violation was cited; there were 4 out of 5 staff who did not have current mandated reporter training. It is recommended to have training renewed every 2 years. See LIC 809D.

A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the center director, Yvonne Ma.

Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

LPA discussed the safe sleep regulations with facility representative and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-and-resources/safe-sleep as an additional resource. LPA also informed facility representative of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Director was reminded that California Law requires licensed Child Care Centers to report unusual incidents or injuries to children in care to child's parents and to the Department of Social Services using the Unusual Incident/Injury form (LIC 624). Incidents must be reported within 24 hours by phone, fax, or electronic mail. LPA informed the Facility Representative that all forms can be downloaded at www.ccld.ca.gov and encouraged the Facility Representative to email childcareadvocatesprogram@dss.ca.gov to be included in the Child Care Quarterly Updates distribution list. The Facility Representative was also reminded that Mandated Reporter Training ("General" and "Child Care Providers") is required for all staff and is to be renewed every 2 years by visiting www.mandatedreporterca.com.
