25 | On June 17, 2026, at 8:00 AM, Licensing Program Analyst (LPA) Elimika Woods conducted an unannounced Annual Random Inspection and met with Director Lolita King-Yates. LPA disclosed the purpose of the inspection, and the facility was toured both indoors and outdoors to conduct a health and safety inspection.
The facility is licensed for 20 infant children. At the time of the inspection, there were two infant children present who were being supervised by one fingerprint cleared staff member. The facility operates Monday through Friday from 7:30 AM to 5:30 PM.
At approximately 8:15 AM, LPA conducted a census of the children in care and verified that the census matched the two signatures recorded on the sign-in sheet. At approximately 9:00 AM, LPA reviewed one child’s file and verified that it contained the required Identification and Emergency Information, Medical Consent form, Immunization Record, and Parent’s Rights documentation.
At approximately 9:30 AM, LPA reviewed one staff file and verified that it contained the required Health Screening Report, Employee Rights, and Statement Acknowledging Requirement to Report Child Abuse forms. LPA also verified that at least one opening and closing staff member possesses a current Pediatric CPR and First Aid certification.
LPA tested the carbon monoxide detector located in the infant room and found it to be functioning properly. The facility is equipped with wired smoke detectors that are connected to notify the Hayward Fire Department when activated. According to the director, the alarm systems are fully operational and serviced regularly. The center maintains a working telephone, first aid kits, pull-down fire alarm stations, and multiple fully charged 3A40BC fire extinguishers, which meet standards established by the State Fire Marshal.
Fire and disaster drills are conducted at least once every six months, with the most recent drill completed on June 8, 2026. All required licensing documents were observed to be posted in a location accessible to parents and visitors.
