13 | On February 13, 2023 at approximately 12:50pm Licensing Program Analyst (LPA) Russ Haderer arrived unannounced to present complaint findings. On arrival, there were 163 children and 27 staff present, the center director Ms. Rania Al-Nazer and administrative staff. LPA toured the facility with the director and confirmed the census and teacher counts. The school is in ratio today.
4. Reporting Party alleged the facility told her that her son hit his head on a pole and it later changed to him hitting his head together with another child at the facility and the facility failed to fully report the incident through the Pro Care Connect Ap. LPA investigated the complaint, reviewed ouch reports and procedures for reporting incidents. LPA determined the child did not hit his head on a pole rather he bumped heads with another child and there was no bruising nor ice needed. LPA found the facility properly called the parents of both children, but failed to complete the final step of reporting through the Pro Care Connect ap. Based on LPAs observations and interviews which were conducted and record review(s), the preponderance of evidence standard has been met, therefore the above allegation(s) is found to be substantiated. Although this incident did happen, the facility is not being cited due to the fact the facility complied with all requirements of notifying parents. |
