On June 14th, 2024 at approximately 1:00pm, Licensing Program Analyst (LPA) April Wright conducted an Annual/Random Inspection and met with Center Director, Mahboob Etemadi. Present during the inspection were eleven (11) preschool age children and three (3) fingerprint cleared staff personnel. The facility is in ratio today The facility was toured for a Health and Safety inspection. Hours of Operation are 7:30am - 5:30pm, Monday - Friday.

CLASSROOMS: Classroom 159 is the preschool age classroom that has adequate age appropriate play and learning materials available. The floors and clean and free of any type of debris. All furniture and equipment are age appropriate and in good repair. There is adequate heating/air conditioning, ventilation and lighting for safety and comfort. Drinking water is available inside and outside of the center. There is proper individual storage space for each child. There is a fully charged 2A10BC fire extinguisher as well as a fully stocked first-aid kit available in the classroom. The isolation area for sick children is a designated area with sign in Room 161 (infant classroom), which is away from other children in care. LPA observed and Center Director confirmed that are no toxins, medicines, cleaning products or hazardous materials visible during today's inspection and were made inaccessible to children in care.

BATHROOMS: The staff's bathroom is separate from the children's bathrooms. All toilets, sinks and faucets are in safe and sanitary operating condition. Children can reach the sinks/toilets and faucets without difficulty and supplies (ex: soap, paper towels) are available to the children.

FOOD SERVICE AREAS: This facility provides breakfast, lunch and snack daily for children in care. There are monthly menus posted at the facility. Kitchen prep areas are neat and orderly with no signs of insects, rodents or vermin. LPA inspected all storage containers for solid waste and observed they have tight fitting covers that are in good condition and repair. Facility has refrigerators which are clean and free of any evidence of rodents, vermin or insects. See LIC809C for continuance.

OUTDOOR PLAY AREAS: The preschool playground is completely fenced, all equipment are age appropriate and free from debris, glass, damage or hazards. There are age appropriate toys and materials for the children. There is a play structure with three (3) slides that has a canopy with foam cushioning to absorb falls and that is anchored for stability. There is a locked shed in the yard and is inaccessible to children in care. Play yard has an additional canopy area to provide shade while children are at play. Per Center Director, there are no weapons or firearms present at the facility. LPA observed and Center Director confirmed that there are no pools, hot tubs or any bodies of free standing water present at the facility. Water play tables have no standing water and are emptied after each use.

RECORDS: All individuals subject to criminal record review have a clearance or exemption and have been associated to the facility. Six (6) children's files four (4) and staff files were reviewed including the center directors file. All staff files have required Health Screening, Employee Rights and all children files contain Identification & Emergency, Personal Rights, and Medical Consent forms. LPA reviewed the facility roster and obtained a copy. All Mandated Reporter Training and CPR/First Aid certificates for staff were reviewed and are up to date. At least one person trained in CPR/First aid is present at the facility when children are present. The center is in compliance with the sign in/out procedure via KinderSign and Daily Name-to-Face Attendance sheets that were verified by the LPA. Disaster drills are being conducted monthly and the last one conducted was on 6/10/2024. All required documents/posters are posted on the family board which is a public accessible area upon entry to the center. At least one person trained in CPR/First aid is present at the facility when children are present.



This facility provides Incidental Medical Services IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. Center Director was reminded that all adults 18 and over, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5 days or, if the penalty is for a repeat violation, for a maximum of 30 days per person will be assessed if this regulation is violated.

Assembly Bill (AB) 2370, Chapter 676, Statutes of 2018, requires all licensed Child Care Centers (CCCs) constructed before January 1, 2010, to test their water (used for drinking and food preparation) for lead contamination before January 1, 2023, and then every 5-years after the date of the first test. For child care center licenses issued after July 1, 2022, the licensee shall test their water for lead within 180 days of licensure pursuant to Written Directives section 101700 (PIN 21-21.1-CCP). LPA verified that the lead testing was completed in accordance to the Written Directives outlined in PIN 21-21.1-CCP. PIN 22-05-CCP.



LPA discussed the safe sleep regulations with the Center Director and discussed the Child Care Licensing Safe Sleep webpage at https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep as an additional resource. LPA also informed licensee of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at https://www.cpsc.gov/ and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Center Director was informed of the MyChildCarePlan.org website; a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.



A notice of site visit was given and must remain posted for 30 days. Exit interview conducted and report was reviewed with the Center Director Mahboob Etemadi.
