On July 20, 2023 at approximately 9:15am, License Program Analysts (LPA) Russ Haderer and Randall Dunevant arrived unannounced to conduct an inspection for health and safety. LPAs met with the site supervisor, Monique Moss. Present upon arrival was 3 teachers and 19 children in care. The hours of operation are Monday to Friday 8am to 5pm. A staff member present did not have fingerprint clearance, a Type A deficiency was issued – see LIC809D.

Facility will be required to publicly post the Type A deficiency for 30 days and provide a copy of the Type A deficiency to all currently enrolled children's parents and collected their signatures and dates on the LIC9224 Acknowledgement of Receipt of Licensing Reports form. For the next 12 months, licensee will be required to provide a copy of the Type A deficiency to parents of newly enrolled children and collect their signatures and dates on the LIC9224 Acknowledgement of Receipt of Licensing Reports form.

The facility on a church campus, independent of the church. The school has separate classrooms (pods) for the children; a small kitchenette with a sink with a microwave and stove, refrigerator and a staff office. Children have a bathroom with separate toilet stalls and a three sinks and a separate private bathroom for staff. There are cubbies available for all children's personal possessions. All hazardous materials and toxins are inaccessible to children in care.

There outdoor play area has play equipment in safe condition and free from sharp, loose, or pointed parts and the areas around or under high climbing equipment has appropriate cushioned material that absorbs a fall. Shade is available and there are no bodies of water accessible to children in care. Records of disaster/fire drills conducted were not available – see LIC809D. All surfaces accessible to children are clean and toxic free. The sign in/out process is managed by Bright Wheel app, all children checked in were present. The snack menu is posted and visible for review. All food/snacks were within expiration date; however, 2% milk was found in the refrigerator (should be 1%). See LIC 809D for deficiency.

Lead testing for consumable water was not been completed, the sampling company is scheduled to come Saturday August 5, 2023. See LIC809D for deficiency. Results of the testing to be publicly posted within 180 days of test results.

The facility is clean and well organized with age appropriate furnishings and equipment in good repair. Surfaces including floors and counter tops are free of hazards and toxins. There is a water fountain for the children available. There are solid waste trash cans with tight fitting lids and cubbies for children to store their belongings.

The facility has fully charged 3A40BC fire extinguishers last serviced January 19, 2022. Smoke alarm system is built into the classroom, last inspected May 23, 2023. Carbon monoxide detector present and tested.

Children’s records were reviewed. Files were reviewed, two children’s files were missing the Physician’s Report – see LIC809D for deficiency. There is one child that required IMS for and EpiPen and an inhaler. Proper forms allowing the center to administer the medications were not available and both medications were expired – see LIC809D for Deficiencies. Staff records were reviewed. Staff members had recently expired Mandated Reporter – a technical violation was issued. Other items missing included Health Screening reports; immunizations; signed employee rights forms - see LIC809D for deficiencies. Transcripts proving educational units were not available for all staff – a technical violation was issue. LPAs reminded site director of the following: Mandated Reporter certificate and CPR/First Aid must be renewed every two years.


Site Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records. The following deficiencies were observed during today's inspection:
