32 | Lead testing for consumable water was not been completed, the sampling company is scheduled to come Saturday August 5, 2023. See LIC809D for deficiency. Results of the testing to be publicly posted within 180 days of test results.
The facility is clean and well organized with age appropriate furnishings and equipment in good repair. Surfaces including floors and counter tops are free of hazards and toxins. There is a water fountain for the children available. There are solid waste trash cans with tight fitting lids and cubbies for children to store their belongings.
The facility has fully charged 3A40BC fire extinguishers last serviced January 19, 2022. Smoke alarm system is built into the classroom, last inspected May 23, 2023. Carbon monoxide detector present and tested.
Children’s records were reviewed. Files were reviewed, two children’s files were missing the Physician’s Report – see LIC809D for deficiency. There is one child that required IMS for and EpiPen and an inhaler. Proper forms allowing the center to administer the medications were not available and both medications were expired – see LIC809D for Deficiencies. Staff records were reviewed. Staff members had recently expired Mandated Reporter – a technical violation was issued. Other items missing included Health Screening reports; immunizations; signed employee rights forms - see LIC809D for deficiencies. Transcripts proving educational units were not available for all staff – a technical violation was issue. LPAs reminded site director of the following: Mandated Reporter certificate and CPR/First Aid must be renewed every two years.
Site Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
This facility provides Incidental Medical Services – IMS. LPA reviewed storage of medication and equipment/supplies, and reviewed children’s, personnel, and administrative records.
