LPAs K. Merchant and D. Campos arrived at Swings and Wings Little Butterflies and met with acting Director Samantha Hernandez for the purpose of conducting a Case Management-Deficiencies inspection. Present were 1 staff and 5 preschool children. On 1/16/2025 during a previous inspection LPAs observed the facility operating the preschool classroom in an unlicensed area (Suite D) which is located adjacent to the licensed facility (Suite E). The facility applied for a capacity increase and room change on 12/2022 which they later rescinded in 6/2023 due to a fire clearance denial. LPAs also observed a drop-in play center operating inside the licensed facility (Suite E) during preschool operating hours. The drop-in play center is open to non-preschool children and their caregivers which results in co-mingling with enrolled preschool children.

LPAs K. Merchant and D. Campos informed acting Director Samantha Hernandez that this report dated 1/27/2025 documents 2 Type A citation(s) which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.

Also, LPAs informed the acting Director to provide a copy of this licensing report dated 1/27/2025 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
