A Prelicensing Visit was conducted today by Licensing Program Analysts (LPAs) Melissa Domantay and Melissa Guirit. LPAs met with Director Sabrina Cazarez. The applicant submitted an application for an infant license. A health and safety inspection was conducted inside and outside. The center has a preschool program on-site (license #013423110). The infant program will operate in 1 Area of the building, in the back right corner behind the play structure. Monday to Friday from 7:30am-6:00pm. The measurements are as follows:

INDOORS: 311.73 square feet = 9 children
OUTDOORS: 289.2 square feet = 4 children

Infant area is equipped with varied age appropriate materials and equipment. The infant program will serve crib-aged infants in Infant area. There is a separate crib area with 5 cribs for crib-aged infants. The diaper changing table is within arms reach of a sink.There is 1 sink available for infants. There is a separate small infant play yard that is separate from preschool children that can be used until it reaches the max capacity of 4 infants, a waiver will need to be submitted for infant play yard area, no more than 4 infants at any given time. The infant yard is fenced all around and has soft turf all around. The staff bathroom will serve as isolation areas for ill children. Separate infant outdoor area has sufficient shading. Infants will only utilize designated infant outdoor area, separate from preschool children. Food will not be provided for infants. There is a backup snack supply bin for children if needed. Facility will utilize electronic sign in/out.

The owner was notified about Incidental Medical Services – IMS. The following information regarding ADA was provided: US Department of Justice (USDOJ) toll-free ADA Information Line at (800) 514-0301 (voice)/ (800) 514-0383 (TTY) and link to publication: Commonly Asked Questions about Child Care Centers and the ADA, available at: http://www.ada.gov/childqanda.htm.


Mandated reporter, appeal rights, civil penalties, unusual incident reporting, fingerprint requirements, and COVID protocols were discussed today. Licensee is also being informed of the web address (www.ccld.ca.gov) for downloading child care forms, and checking Provider Information Notices (PINs). The director is encouraged to email ChildCareAdvocatesprogram@dss.ca.gov to be included in the Child Care Quarterly Updates distribution list. The director is also reminded that mandated reporter training is required for all staff and is to be renewed every 2 years at www.mandatedreporterca.com.

Zero Tolerance policies were explained. The center is found to be clean, safe, sanitary, and in good repair.

A fire clearance for 45 children including 9 infants was received from Alameda City Department on 2/26/2021. Facility is equipped with a 3A40BC fire extinguisher, fire alarm, centralized smoke and carbon detection system, working telephone, and first aid supplies.

The facility will be licensed as of today 9/3/2021.

An exit interview was conducted.
