25 | On 05/25/2022 at 12:00PM, Licensing Program Analyst (LPA) Kelly Phan arrived at for an unannounced required inspection, and met with Licensee Bisma Shah. Present for this inspection was the licensee, licensee's husband, their own two children (7 1/2 and 12), along with 4 preschool aged children. The home was toured with the licensee to conduct a health and safety inspection. Hours of operation for day care are Monday through Friday, 8:00am to 6:00pm.
ON LIMITS: hallway bathroom #1, downstairs backyard, dining room, living room, kitchen, bedroom #1 closest to the hallway bathroom; isolation area is kitchen
OFF LIMITS: garage, bedroom #2 (children's room), bedroom #3 (master bedroom), master bathroom #2, and front patio. Off limit areas are inaccessible by closed and/or locked doors and visual supervision.
The home is duplex, which is neat and clean, with heating and ventilation for safety and comfort. At 12:20PM, LPA toured the downstairs backyard along with licensee's husband; he states that the backyard area is not shared with other tenants; LPA advised that licensee would need to have full supervision while playing outside as it is not fully fenced. There were ample age appropriate toys that were observed to be safe and in good condition. There were no toxins, medicines, and hazardous items during today's inspection. There is a fully 2A10BC fire extinguisher, a dual carbon monoxide and smoke detector, and telephone. The home does not have a fireplace, per licensee they have central heating. Per licensee, there are no firearms or any bodies of water in the home; the licensee does have a small bird. The licensee conducts and documents fire drill log indicates a drill was conducted 02/03/22. Required licensing documents are posted and visible for public review, except the PUB 394 (Notification of Parent Rights poster) was not present. LPA informs licensee that it is a required poster at her facility. LPA went ahead and provided a copy to licensee.
