On 12/06/2024 at 8:30 AM Licensing Program Analyst (LPA) Janai McClain met with Licensee Tonja Primas for an Unannounced Case Management Inspection. Present during the inspection were one infant and six preschool age children in care. The facility operates Monday-Friday 7:30 AM-5:30 PM.

LPA toured the facility to conduct a Health and Safety inspection. LPA observed three fish tanks. Fish tank (1) is on a stand making it inaccessible to children. Fish tank (2) is in the off-limits bedroom near the kitchen. Fish tank (3) is on a low level stand and secured with clips. Licensee will purchase a stand for fish tank (3) or move it to an off-limits area.

No deficiencies were cited during the inspection.

Exit interview conducted and report was reviewed with the Licensee Tonja Primas. Appeal Rights provided.

Notice of Site Visit must remain posted for 30 days.
