25 | On 09/17/2024 at 9:15 AM Licensing Program Analyst (LPA), A. Curry met with Licensee, Tobias Riday-White, for an unannounced Case Management- Licensee Initiated Visit. Licensee submitted an application to increase capacity to a large license with a capacity of 14. Also present during the inspection were licensee's fingerprint cleared assistant and 5 children in care consisting of 3 infants and 2 preschoolers. Licensee's home was toured for a health and safety inspection. The facility plans to operate Monday - Friday 8:00AM - 5:00PM.
The home is single-story and consist of 3 bedrooms, 2 bathrooms, living room, kitchen, dining area, garage, and backyard. All off limit areas in the home will be made inaccessible with closed doors, safety gates, and locks. The licensee was advised that any area that will be changed to an on limit area will need to be inspected by Licensing prior to use. The home was observed to be neat and clean with age appropriate materials for children. All toxins, cleaning products, medications, and hazardous materials were inaccessible during today's visit. The home is equipped with a fully charged 3A40BC fire extinguisher and working dual smoke alarm/carbon monoxide detector.
ON LIMITS AREA: Living room, kitchen, dining area, first and second bedrooms to the left, bathroom in the hallway, and backyard, which consist of 3 separately fenced sections. There are 2 Alpacas in the far back section of the yard. The licensee was advised that 100% visual supervision of children in the backyard is required.
OFF LIMITS AREA: Master bedroom, master bathroom, and garage.
ISOLATION AREA: An area in the living room.
Approved fire clearance was received on 07/26/2024.
Increase of capacity has been approved and is effective as of today, 09/17/2024.
Notice of site visit was given and must remain posted for 30 days.
Exit interview conducted, appeal rights were given, capacity for large license was discussed, and report was reviewed with Licensee, Tobias Riday-White.
