25 | Licensing Program Analyst Lisa Dyer conducted an announced pre-licensing inspection due to a location change. Present was the applicant and one preschool child.
Facility Location: At 9:45 a.m., the following was observed: The home was toured and consists of 6 bedrooms, 2 bathrooms, living room, dining room, family room and kitchen. Applicant will be utilizing (downstairs) the living room, dining room, two downstairs bedrooms and the bathroom for her day care area. Off limits areas are the family room, kitchen and entire upstairs: 4 bedrooms and one bathroom. These areas will be inaccessible by closed and/or locked doors; door knob covers; gates and visual supervision.
Facility Review: Home is neat and clean, with heating and ventilation for safety and comfort. Children will sleep on cots. It was discussed that infants will need to sleep in cribs or pack 'n plays. There are toys and play space available. There is child sized furniture. Stairs are blocked with child gates. Isolation area for sick children will be in the downstairs bedroom. There are no toxins accessible today. There is a 3-A:40-B:C fire extinguisher, smoke detector (tested), carbon monoxide detector (tested) and a first aid kit. Per applicant, there are no firearms or bodies of water on the premises. Applicant has two water fixtures in the dining room. Fixtures are filled with rocks so water cannot pool at the bottom. These are not to be used during day-care hours. Both fireplaces are screened. She has a cell phone and a home phone. Backyard is not safe for child play and will not be licensed. Children will play in the front yard. Pets consist of 2 bulldogs, 1 yorkie, 1 cocker spaniel, 1 pit bull, and fish. Adults that live in the home are fingerprint cleared.
Training: Applicant has completed Preventative Health Training on 12/11/2021. CPR/First Aid Training expires 10/31/22. Mandated Reporter Training certificate expires 2/22/24. The applicant's husband is the owner of the property, and verification was viewed.
Technical Assistance During Inspection: LPA reviewed with applicant the LIC 311D, Forms/Records To Keep In Your Family Child Care Homes, children's forms/records, facility forms/records, and information to be posted. Incidental Medical Services (IMS) policy was discussed. (continued) |
