On 7/14/23, Licensing Program Analyst (LPA) Melissa Guirit conducted a Prelicensing inspection for the initial application that was submitted by the applicant. LPA met with applicant, Bessie Sholes and toured all areas of the facility for a health and safety inspection. School Age component is in the same building as the Infant and Preschool components. The school age children will utilize one classroom in the back of the facility. The hours of operation are Monday through Friday from 6:00am to 7:00pm. The total measurements are as follows:

INDOORS: 594.91 sq ft = 17 children
OUTDOORS : 4,333.81 sq ft = 57 children
A fire clearance, with a total capacity of 6 infants, ages birth to 2 years old, 23 preschoolers, ages two years to first grade entry, with a toddler option of 15 toddlers, ages 18 months to 36 months, and 12 school age children ages 4.9 to 12 years was received from the Oakland Fire District on 05/01/23.

LPA observed that the classroom has sufficient lighting and appears to be clean and in good condition. LPA observed an ample supply of age appropriate activities, equipment and furniture. There is one designated toilet and one sink for school age children to utlize. The building has a fully charged 3A40BC fire extinguisher, working fire alarm and carbon monoxide detector, and first aid supplies. There is a working telephone at the center. LPA did not observe any hazardous items that would be accessible to children. Families will provide food from home and facility has emergency snacks that will be available for children. Facility will utilize electronic sign in/out.

LPA observed the outdoor play area, which is fully fenced. LPA reminded applicant to be sure that 100% supervision is provided. The school age children will be sharing the play yard with the infant and preschoo/toddler components on a rotating schedule. The play area provide sufficient cushion for children in case of a fall. LPA observed shaded areas for children. The isolation area will be in the main office. See 809-C for continuance.

Kristen Davis is considered a qualified Director with current CPR/First Aid that expires 12/2023 and mandated reporter training's completed and current.

Applicant was reminded that all adults 18 and over responsible for administration or direct supervision of staff, persons who provides care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.71, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see PIN 22-02CCP. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. LPA reviewed with applicant the LIC 311A, Records to Be Maintained At The Facility, for child’s records, personnel records, administrative records, and documents to be posted.

LPA discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep, as an additional resource. LPA also informed [applicant, licensee, or facility representative] of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California.
