On 10/3/23 at 9:30am, Licensing Program Manager (LPM) Loretta Dyson and Licensing Program Analyst (LPA) Brittany Crass arrived at the home for an announced prelicensing inspection. The applicant submitted an application for a change of location. LPM and LPA met with Rhonda Glenn. There was no one else present in the home. The facility will offer care for children 24 hours a day. The applicant understands that no one child can be in care for 24 hours. The applicant will provide care for children of all ages.

A tour of the home was conducted for a health and safety inspection.The home is a single story unit in a duplex. The home consists of the living room, kitchen, 3 bedrooms and 1 bathroom. The home is neat and clean with sufficient heating and ventilation for the safety and comfort of children in care. All areas of the home are on limits to children. The kitchen cabinets are secured with locks to prevent access to items that could be a danger to children. LPM advised the applicant to keep the door in the middle bedroom, leading outdoors, closed and locked so that children are not able to get out of the home without supervision. The isolation area will be in one of the bedrooms, away from other children in care. There is an ample supply of age appropriate toys and activities for children, and they appear to be in good condition and safe.

There is a working combination carbon/smoke detector, working telephone, first aid supplies, and a fully charged 3A40BC fire extinguisher. The wall heater in the living room is blocked to prevent access by children. The applicant advised that she will use the local park for outdoor play. The applicant was reminded to have 100% visual supervision of children when away from the home. LPM and LPA did not observe any bodies of water, poisons, or hazardous items that would be accessible to children today. The applicant stated there are no firearms in the home. The applicant completed the preventative health and safety training which includes 1 hour of nutrition. The applicant completed the CPR and First Aid training and was issued a certificate that expires in 1/9/24. The licensee has completed the mandated reporter training, and it expires on 1/9/24.

LPM reviewed a fils that the applicant has, from a child previously in care, and verified that the applicant has all required forms. LPM reminded the applicant to ensure that all sections of the forms are completed. The applicant has a board, for required postings, which is on the wall right inside of the front door. LPM reminded the applicant that parents/public need to have access to this board during hours of operation.

LPM reviewed children's personal rights and the applicant advised that she understands that children's personal rights should not be violated and no corporal punishment. LPM discussed supervision of children, capacity/ratio options, transportation of children, and requirements for an assistant/substitute. LPM reminded the applicant that smoking, baby walkers, bouncers, jumpers and similar items are not allowed in family child care homes. LPM reminded the applicant of their responsibility to ensure that the immunization records are up to date and have discussions with parents/guardians if they are not. LPM advised that it does not need to be done if children are enrolled in school. LPM reminded the applicant to report any injuries requiring medical attention or unusual incidents to the Oakland Regional Child Care office. LPM reviewed the form to follow up in writing within 7 days of the injury/unusual incident. The applicant was encouraged to periodically review regulations, guidelines and PINS on the website www.ccld.ca.gov.

Applicant was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, except as specified in Health and Safety Code section 1596.871, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day for a maximum of 5-days or, if the penalty is for a repeat violation, for a maximum of 30-days per person will be assessed if this regulation is violated. LPM verified that the applicant has a criminal record clearance and are associated to the license.

The applicant provided a copy of their lease as proof of control of property.

Because the applicant rents the home, proof of landlord notification is required. The LPM observed the Property Owner/Landlord Notification form (LIC9151) that the applicant confirms was provided to the property owner/landlord. The applicant obtained a signed Property Owner/Landlord Consent form (LIC 9149).

LPM reviewed the LIC 311D, Forms/Records to Keep In Your Family Child Care Homes, children’s forms/records, facility forms/records, and information to be posted. Entrance Checklist was provided to the applicant. LPM discussed the safe sleep regulations with applicant and discussed the Child Care Licensing Safe Sleep webpage at: https://www.cdss.ca.gov/inforesources/child-care-licensing/public-information-andresources/safe-sleep, as an additional resource. LPM also informed applicant of the importance of checking for recalled infant devices on the United States Consumer Product Safety Commission (CPSC) website at: https://www.cpsc.gov/, and recommended they register all infant devices with the CPSC to be notified of any recalls on their purchased equipment.

On 9/8/2023, the California Attorney General - Megan’s Law website was searched for information on sex offenders required to register with local law enforcement under California's Megan's Law. No registered sex offenders were found at the facility address. Under state law, some registered sex offenders are not subject to public disclosure; therefore, they may not have been included in this search. However, the Department conducts a monthly cross reference of each address on record for all registered sex offenders against all CCLD facility addresses pursuant to information shared by California DOJ.

Applicant was informed of the MyChildCarePlan.org site, a consumer education website that helps families obtain child care by connecting them to child care providers and Resource and Referral Agencies (R&Rs) throughout California. Community Care Licensing Division (CCLD) regularly sends information to licensed facilities, providers, and stakeholders by way of Provider Information Notices (PIN), Program Quarterly Update Newsletters and other important information communication platforms. The home will be licensed effective today, 10/3/23. An exit interview was conducted with the applicant, Rhonda Glenn, and a copy of the report was provided.
