32 | Licensees’ Pediatric CPR and First Aid training expires 2/23/2026. The Licensee's Assistant, Bridgett Ekow's CPR certificate was not EMSA approved, which is a Type B violation. The Licensee’s Mandated Reporter expires 1/29/2026. All required forms are posted and visible for public view on the wall by the front door of the home.
Two Type A citations were issued for personal rights and safe sleep. One Type B citation was issued for Bridgette's CPR certification. See LIC 809-D for deficiencies.
LPAs informed the Licensee to provide a copy of this licensing report dated 3/7/2024 that documents the Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Licensee was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.
Due to multiple citations being issued during todays visit, the capacity increase will remain in pending status until all citations have been cleared.
Exit interview conducted and report was reviewed with the Licensee Rhonda Glenn.
Appeal Rights provided.
Notice of Site Visit must remain posted for 30 days.
