On 3/7/2024 at 12:55pm Licensing Program Analysts (LPAs) Janai McClain and Indira Loza met with Licensee Rhonda Glenn for an Unannounced Capacity Increase Inspection. Present during the inspection were one infant, and six preschool aged children, and one assistant, Bridgette Ekow. The Licensee arrived about 20 minutes after LPAs arrived. The Licensee’s home was toured for a health and safety inspection. The facility operates 24 hours a day, 7 days a week.

The facility is a single story unit in a duplex. The home consists of a living room, kitchen, 3 bedrooms, and 1 bathroom.

ON LIMITS AREA: All areas
OFF LIMITS AREA: None
ISOLATION AREA: last bedroom on the right near the bathroom

At 12:57pm LPAs observed a two year old child sleeping in a car seat with the harness on which is a Type A violation. Then at approximately 1:13pm LPAs observed an infant placed to sleep in another car seat with the harness on which is also a Type A violation. LPAs also observed the infant being transferred to a cot with a blanket.

The inside of the home was observed to be neat, clean with ample age-appropriate materials for the children. All toxins, cleaning products, and hazardous materials were observed to be in inaccessible areas. Licensee provides food for the children that is properly stored and maintained. All cribs and napping equipment are clean, well maintained and free from defects. Licensee stated there are no firearms and no pets in the home.

Licensees’ Pediatric CPR and First Aid training expires 2/23/2026. The Licensee's Assistant, Bridgett Ekow's CPR certificate was not EMSA approved, which is a Type B violation. The Licensee’s Mandated Reporter expires 1/29/2026. All required forms are posted and visible for public view on the wall by the front door of the home.

Two Type A citations were issued for personal rights and safe sleep. One Type B citation was issued for Bridgette's CPR certification. See LIC 809-D for deficiencies.

LPAs informed the Licensee to provide a copy of this licensing report dated 3/7/2024 that documents the Type A citations to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

Licensee was reminded that all adults 18 and over living in the home, persons who provide care and supervision to children, and staff who have contact with children, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a licensed Family Child Care Home. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Due to multiple citations being issued during todays visit, the capacity increase will remain in pending status until all citations have been cleared.

Exit interview conducted and report was reviewed with the Licensee Rhonda Glenn.

Notice of Site Visit must remain posted for 30 days.
