On July 10, 2024 at 8:47 am Licensing Program Analysts (LPAs) Janai McClain met with Licensee Rhonda Glenn for a case management visit regarding another matter.

Upon arrival LPA observed one assistant with four preschool age and six school age children in care causing the facility to be out of ratio. LPA observed that the cabinet under the kitchen sink was unlocked, making cleaning compounds accessible to children in care.

LPA Janai McClain informed licensee Rhonda Glenn that this report dated 7/10/24 document(s) two Type A citation(s) which shall be posted for 30 consecutive days as there is/are immediate risk(s) to the health, safety, or personal rights of children in care.

Also, LPA Janai McClain informed the licensee Rhonda Glenn to provide a copy of this licensing report dated 7/10/24 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.

A copy of the LIC9224 is being provided during the inspection.


The licensee gave LPA permission to review the report with assistant Bridgette Ekow.
Exit interview conducted. Report and Appeal Rights provided.
Notice of Site visit must remain posted for 30 days.
