13 | On 12/9/2021 at 11:00AM, Licensing Program Analyst (LPA) G. Luk arrived unannounced to deliver complaint findings for the allegation above. LPA met with Executive Director, Dolly Rizvi and explained to her the reason for the visit.
During the course of the investigation, the Department conducted interviews with staff, residents, witness, and complainant. Resident’s physician’s report, care plan, care notes, photos, incident reports, medical records, and hospital discharge records were obtained and reviewed.
Based on the Department’s investigation, R1 was sent to the hospital on 6/23/2021 after R1’s conservator discovered the open wound and instructed facility staff to do so. R1’s care plan dated 6/15/2021 stated that R1 required two-person assistance with bathing twice a week and two-person assistance with toileting needs. Staff denied seeing an open wound on R1’s leg prior to hospital visit despite providing diaper changes, bathing, and dressing R1. (Continue on LIC9099C...) |
