32 | There were Three (3) deficiencies were cited for today's inspection
Type B: at 10:40AM, LPA also reviewed her staff files and found it to be incomplete as it is still missing her immunization records. Staff member states that she has an appointment scheduled to obtain her medical statement. Type B deficiency was cited. See LIC 809D
Type B: also at 10:40AM, LPA also observed an immunization summary mixed with sleep logs, which belongs to a child who had missing immunizations during last annual visit on 02/25/2022; licensee states that the mother states the child shall receive the rest of the vaccines once she reaches 5 months old. LPA also verified with staff member who states the child is about 3 to 4 months old right now. SEE LIC 809D.
Type A: At 11:20AM, LPA verified with licensee that one of the child is five years old and half and supposed to be attending transitional kindergarten (TK), but she missed the age and were not enough space in TK program, and her mother wanted to place her at her day care. LPA informed licensee that her license only allows for 4 infants; six children who are no more than three of whom may be infants; and if more than six children, the two additional children shall be enrolled and attending kindergarten or elementary school and at least six years old. Type A deficiency is cited. See LIC 809D.
LPA Kelly PHAN informed licensee Sahar that this report dated 04/06/2022 document(s) 1 Type A citation which shall be posted for 30 consecutive days as there is immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Kelly PHAN informed the licensee to provide a copy of this licensing report dated 04/06/2022 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Appeal rights and a notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.00. Exit interview conducted and report was reviewed with licensee Sahar Eldeeb.
