On 04/29/2022, approximately at 1:00PM Licensing Program Analysts (LPAs) Kelly Phan and Russ Haderer arrived unannounced and met with licensee Sahar Eldeeb for the purpose of conducting a plan of correction inspection to verify if corrections were made. Present for this inspection was the licensee, housekeeper (she comes once a week for few hours and is not alone with children at anytime),5 preschool aged children and 1 infant in care. LPAs entered the facility and began the inspection of the facility to conduct a health and safety check.

ON 4/06/2022 the following deficiency was cited: LPA observed 7 children in care, one was not at least six years old and were not attending school, therefore a type A deficiency was cited. Based on LPA's observations today, licensee has failed to correct deficiency cited from prior inspection to reduce capacity by two (2) children. A civil penalty of $250.00 was cited, see LIC 809D.

During the inspection today LPAs toured the entire facility and verified the ages of the children with licensee and observed to have 6 children (five preschoolers and 1 infant) in her care.

As of today, the licensee is in ratio today and complying with the capacity stated on her small family child care license. This report shall be kept on file for 3 years.
