25 | On November 7, 2024 at 10:45AM, Regional Manager (RM) Loretta Dyson, Licensing Program Manager (LPM) Jason Jang, Licensing Program Analysts (LPAs) Jaleesa Jackson, and Morgan Pringle met with Licensee Sahar Eldeeb at the Oakland Southeast Regional office for a Noncompliance Conference (NCC). Also present was the Licensee's son and attorney. The purpose of this NCC was to discuss the facility noncompliance regarding lack of supervision, ratio, infant safe sleep and adults in the home without a criminal record clearance.
Within the last 12 months, this facility has had five (5) Type A violations in regards to the facility not following infant safe sleep, three (3) Type A violations for criminal record clearance, one (1) Type A violation for ratio, and one (1) Type A violation for lack of supervision. The facility has also received multiple Type B violations in the last 12 months.
During the meeting, RM Dyson and LPM Jang discussed with the Licensee the reasons for the non-compliance with the regulations. The current hours of operation and how ratio will be maintained with 24 hour care. LPA reminded Licensee that care provided during the day and overnight shall not exceed 24 hours from when the child entered the care. The Licensee is being provided information on our Technical Support Program (TSP) and being placed on required visits for the next 12 months.
LPA Jackson informed the Licensee that the licensee is to provide a copy of this licensing report dated 11/7/2024 that documents a Noncompliance Conference to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
