On Wednesday, September 18, 2024, at approximately 2:00pm Licensing Program Manager Chandra Charles and Licensing Program Analyst April Wright met with Licensee Ysamel Falcon, at the Oakland Southeast Regional Office for an Informal Conference meeting. The purpose of this Informal Conference is to discuss, the existing deficiencies, any current problem areas in the operation of the licensee’s Family Childcare Home facility.

On August 30, 2024, Licensing Program Analyst April Wright, conducted an Annual/Random site inspection of the licensee’s family childcare home. During the entire site inspection it was observed the licensee Ysamel Falcon, was not present at the facility. Care and Supervision of (10) children was strictly upon Monkia Rodriguez. After the arrival of the Licensing Program Analyst, assistant Angelica Llamas De Flores, arrived. Later during the site inspection Ysglee Rodriguez (another day care provider arrived). No one provided an explanation on 08/30/2024, why the licensee of the facility was not present.

The following deficiencies were discussed with the Licensee Ysamel Falcon.

· Operation of a Family Childcare home – Title 22 regulation #102417(a)

· Staff Ratio and Capacity – Title 22 regulation #102416.5

· Children records keeping – Title 22 regulation #102425(j)(2)(d)

Licensee was presented with the following documents:

· FCCH capacity breakdown sheet.

· Title 22 regulations regarding Staff ratio, children record keeping, Operation of a FCCH regarding licensee’s presence at the facility, and Inspection Authority of the Department.

After reviewing licensees facility history, and going over Title 22 Regulations, at this time the decision was made not to be placed on required visits. Report was read and reviewed with Licensee Ysamel Falcon.
