On January 27, 2023 at approximately 8:30am, LPA Haderer arrived unannounced for an annual inspection for compliance to Health and Safety. Present for the inspection was the site director Nithya Prabhakaran, her fingerprint and associated assistant Josephine Lawrence and 8 children in care (3 to 5 years in age) The hours of operation are 7:00 to 6:00pm.

The school is operated on the property of an historical home (now the administrative building) and in a newly constructed stand-alone purpose-built child care center building in the back. In the childcare center there are 3 classrooms: Explorer; Achievers; and Performers room; in the administration building there is a Library and STEM room.



LPA observed that all preschool rooms are clean and safe for all children and staff. Children bring their own bottled water from home, filtered water is available should the children run out. Water is brought outside in the warmer summer months. Children bring their own lunch; snacks are provided by the facility. Snack food and beverages were checked and not expired. All food and beverages that require refrigeration are stored in covered containers at 36 degrees F. The facility has a kitchen and no cleaners or toxic materials were observed stored with food. The Director's office and staff bathroom will serve as isolation areas for sick children.

The facility is clean and well organized with ample age appropriate furnishings and equipment. Surfaces including floors and counter tops are clean and toxic free. Heating and ventilation is acceptable in the buildings. There were no hazardous items/toxins observed to be accessible to children in care today. There are no bodies of water accessible to children in care. First aid supplies are available in the center.

There is a children’s bathroom with 4 toilets and 3 handwashing sinks. There are also additional sinks with hot and cold taps available in each classroom and the outside play area. There are enough towels and soap supplies. All toilets, handwashing and cleaning areas are in safe and sanitary operating condition. There are solid waste containers with tight-fitting lids.

LPA observed sufficient cots for the children. Site director stated that bed linens are taken home and washed by parents each week (if there is an accident, they are washed that day). There is a separate staff bathroom located in the administration building, not used by children. Director stated that there are no weapons or firearms on the premises.



Each classroom has fully charged 2A10BC fire extinguishers located by the exits. The last annual inspection was done 11-03-2021. Carbon monoxide detectors were available, tested and working. The building has a fire alarm system, inspected and passed by the Fremont Fire Department. Disaster drills are conducted at least once every 6-months, the last drill was conducted 9-06-2022.

The playground areas are completely surrounded by a 6-foot fence. Playground area has a waiver to limit the number of children to a maximum of 31 at a time. All equipment is in safe condition and free from sharp, loose or pointed parts and the areas around or under high climbing equipment has appropriate rubber cushioned material that absorbs a fall. Ample shade is available for the children in the play areas and teachers are always present. Personal water bottles travel with the children to the outside area, all water bottles are labeled with each child’s first and last names.

The sign in/out sheets were reviewed and are accurate.

Children's records were reviewed: LPA requested and reviewed facility roster. All files selected were complete and up to date. Children’s files were missing the Physician’s Reports, see LIC809D for deficiency. Staff files were reviewed. Opening and closing staff have current CPR and first aid training. All staff subjected to criminal review have been cleared and associated to the facility. All files were complete, up to date and found to be in compliance with Title 22 regulations. Immunization records were available showing all teaches files were complete.

All documents required to be posted were appropriately posted on the walls: License; Emergency Disaster Plan; Earthquake Preparedness checklist; Notification of Parents Rights; Personal rights; Child seatbelt laws; menus; daily activity schedules. There is one waiver for this facility for playground space to be alternated to limit the number of children. Waiver was publicly posted.

Site Director was reminded that all adults 18 and over, including employees and volunteers, must obtain a criminal record clearance or exemption, or transfer their existing clearance or exemption, prior to initial presence in a Child Care Center. A civil penalty of $100.00 minimum/day up to $500.00 maximum per day/per person will be assessed if this regulation is violated.

Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual – Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department. There was one deficiency issued, children’s files were missing the physicians Reports - see LIC809D for deficiency.



A notice of site visit was given and must remain posted for 30 days.
