13 | On March 20, 2026, at 8:00 AM, Licensing Program Analyst (LPA) Elimika Woods conducted an unannounced visit to conclude a complaint investigation regarding the above allegation. LPA met with Director Jackie Amo-Mensah. Present during the inspection were three fingerprint-cleared assistants and 21 preschool-age children.
LPA conducted observations, interviews, and reviewed relevant documentation. Based on the information obtained, LPA was unable to substantiate the allegation that a child sustained an unexplained injury due to staff neglect or lack of supervision. During the course of this investigation, LPA did not observe any deficiencies in supervision practices during visits, nor were there any records or documentation reviewed that supported the allegation of an unexplained injury resulting from staff neglect or lack of supervision.
Based on interviews, documentation reviewed, and observations conducted, the allegation that a child sustained an unexplained injury due to staff neglect or lack of supervision is determined to be UNSUBSTANTIATED, meaning that although the allegation may have occurred or may be valid, there is insufficient evidence to determine whether the alleged violation occurred.
An exit interview was conducted, and appeal rights were discussed with Director Jackie Amo-Mensah. |
