On 06/272025 at 10:30am, Licensing Program Analyst (LPA) Christina Uribe conducted an unannounced visit for the purpose of investigating a complaint regarding the above allegation of a physical plant violation and met with assistant director, Viradel Porciuncula. Also present at the time of today’s inspection were 3 staff and 26 children.

LPA Uribe conducted interviews, obtained relevant documents, and made observations in the facility. Based on evidence received which was reviewed, the preponderance of evidence standard has been met, therefore the above allegation that there are cockroaches, ants, spiders, and rat droppings in the facility is SUBSTANTIATED.California Code of Regulations, Title 22, Division 12, Chapter 1, Article 06, Section 101238(a)(1) - Building & Grounds is being cited on the attached LIC 9099D.

Report was reviewed and a notice of site visit was given and must remain posted for 30 days. Appeal rights were given. Exit interview was conducted with assistant director, Viradel Porciuncula.

101238(a) The child care center shall be clean, safe, sanitary, and in good repair at all times to ensure the safety and well-being of children, employees, and visitors. (1) The licensee shall take measures tp keep the center free of flies, other insects, and rodents.

The facility staff will ensure that all furniture, equipment, and physical space, including those used for storage, within the classrooms are kept clean and free of pests and rodent droppings. Facility staff will also create a written plan of action for maintaining the cleanliness of the facility from pests and

This requirement was not met as evidenced by: LPA Uribe observed rodent droppings and deceased insects inside of storage tubs inside of the classroom. Additionally, evidence was obtained that the facility has an intermittent issue with keeping the facility free of pests.

rodents, and a plan of action to ensure that preventative measures are regularly taken to keep the facility free of pests and rodents. Director will email these written plan of actions to LPA Uribe at christina.uribe@dss.ca.gov no later than the due date of 07/18/2025.
