Licensing Program Analyst (LPA) Melanie Otsuji arrived to the facility unannounced to conclude investigation into the above allegation. LPA was met by owner, Sugam Tiku and Director, Nirupama Bajpai. Also present during today's visit were an three additional staff members and 20 preschool aged children.

During the course of the investigation LPA conducted record review, made observations and conducted interviews. Based on the information gathered, it was stated that children were allowed to go outside on the play yard prior to staff ensuring that the yard was clean and ready for children. Based on information gathered, it had rained in the days leading up to this incident and staff did not have the opportunity to clean up the play yard prior to allowing children access. Based on LPAs observations and record review(s), the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. Health and Safety code 1596.955 is being cited on the attached LIC. 9099D.
A notice of site visit was given and must remain posted for 30 days.
Appeal Rights were given and discussed. An exit interview was conducted.

Although the allegation of staff did not ensure bedding was individually stored may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation did or did not occur, therefore the allegation is unsubstantiated. Exit interview conducted with Director, Nirupama Bajpai. A Notice of Site Visit was provided and must remain posted for 30 days.
