13 | Licensing Program Analyst (LPA) Jyoti Saini arrived unannounced to deliver the findings from a complaint investigation for the above allegation. LPA met with director Gabriela Arrizon and explained the purpose of the inspection. Present for today's visit, there was director, and 18 staff members supervising 145 children.
Based on interviews and record review, the facility acknowledged that an incident occurred in which Child 1 (C1) sustained an injury. Information gathered revealed that staff were supervising children in the playground however, no staff observed the moment the injury occurred. Staff became aware of the injury only after the child approached or was brought to them. Although the injury did not require immediate medical attention and parents were notified the same day, the lack of direct observation indicates that supervision was not maintained at all times, as required by Title 22.The preponderance of evidence standard has been met; therefore, the allegation is found to be SUBSTANTIATED. California Code of Regulations, Title 22, Division 12, Chapter 1, Section 101229(a)(1) is being cited on the attached LIC 9099D.
Notice of site visit was provided and must be posted for 30 days.
Appeal rights were given, and an exit interview was conducted with director Gabriela Arrizon.
