On February 12, 2025, Licensing Program Analysts (LPA) Randy Miranda met with Director Shawnnen Frasier to deliver the findings from a complaint investigation for the above allegation. Present during the inspection were the two directors, two teachers, and twenty three (23) children in care (eight-2year olds, seven-3year olds, and eight-4year olds).

During the course of the investigation LPA reviewed children’s files and conducted staff interviews. By way of admission, there was an incident that occurred in the facility which was not reported to the parents and Licensing. The preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 12, Chapter 1, Section 101212(d)(1)(C) is being cited on the attached LIC 9099D).

A notice of site visit was given and must remain posted for 30 days. Appeal Rights provided and discussed.
An exit interview was conducted with Director Shawnnen Frasier.

Based on interviews, record reviews, observations, and video footage, the allegation stated above may have happened or is valid, there is not a preponderance of evidence to prove the alleged violation(s) did or did not occur, therefore the allegation is UNSUBSTANTIATED.

A notice of site visit was given and must remain posted for 30 days. Appeal Rights provided and discussed.

The Facility shall watch the "Child Care Reporting Requirements" at www.ccld.childcarevideos.org and submit a written statement explaining when and how an event should be reported to the Community Care Licensing Division (CCLD) by the POC due date of 02/27/2025.
