25 | On 07/15/2026 at 8:10am, Licensing Program Analyst (LPA) Jialing “Julianne” Zhu met with applicant Bhavnaben Shah for an announced Change of Location prelicensing inspection. Present during the inspection were applicant and fingerprint-cleared spouse. The applicant lives in the home with spouse. A Prelicensing Entrance Checklist (LIC 9280) was provided to the applicant. The home was toured to conduct a Health and Safety Inspection. The facility’s intended hours of operation are Monday-Friday from 8:30am to 5:45pm. The intended ages of children to be served are 10 months to 10 years old.
The home is a single-story home with three (3) bedrooms, two (2) bathrooms, living room, family room, kitchen, attached garage, side yard, and backyard. LPA did not observe any pools, hot tubs, or similar bodies of water on premises.
The on-limit areas are the family room (main daycare area), middle hallway bedroom (Bedroom 2), hallway bathroom (across from Bedroom 2), and side yard. The off-limit areas are the primary bedroom (Bedroom 3) and bathroom, back hallway bedroom (Bedroom 1), living room, kitchen, garage, and backyard, which will be inaccessible by closed and/or locked doors and visual supervision. Living room will be used as walkthrough only with visual supervision to access the family room. The isolation area is Bedroom 2. When a child shows signs of illness, he/she will be separated from other children here.
The inside of the home is observed to be clean and orderly with central heating, cooling, and ventilation for safety and comfort. LPA observed there are ample safe and age-appropriate toys, play equipment and materials. All toxins, cleaning products, and hazardous materials have been made inaccessible to children. There is a fireplace in the living room, which has been made inaccessible to children with screen doors.
