On 4/21/2022 at 7:54AM, Licensing Program Analysts (LPAs) C. Lin and L. Fici arrived unannounced to conduct an initial 10-day complaint investigation in regards to the allegations above. LPAs met with Administrator, Emilissa Pangilinan and explained the purpose of the visit.

During investigation, a full basket of medications for R1 was placed on the top of medication cabinets in the dinning area which were accessable to other clients. The Department conducted interviews with Administrator and S1, both of them admitted that was a mistake of leaving medications unlocked.

Based on LPA’s observations and interviews which were conducted, and record review(s), the preponderance of evidence standard has been met, therefore the above allegation is found to be SUBSTANTIATED. California Code of Regulations, (Title 22, Division 6, Chapter 8), are being cited on the attached LIC9099-D. Failure to correct deficiency by POC date may result in additional Civil Penalties.

Exit interview conducted with Administrator, LIC9099D, Appeal Rights and a copy of this report provided.

Staff locked up medication during visit.
In addition, Administrator agreed to retrain staff for the regulations and submit training agenda and staff sign-in sheet to CCL by the POC due day.

Based on observation, the licensee did not comply with the section cited above, where medication being accessible which poses an immediate health, safety or personal rights risk to persons in care.
