On 6/17/2020 Licensing Program Analysts (LPAs) Y. Flores-Larios and A. O'Hollaren conducted an announced tele-visit with Deborah Savoie, Executive Director.

Facility currently has 9 residents that have tested positive for COVID-19 and currently isolating on the second floor in individual apartments except for two couples. The purpose of this tele-visit is to review infection control procedures that the facility has implemented. Facility has a total of 6 floors with individual apartments. First floor consists of main kitchen, dining room, private dining, and other personal care services such as salon, library and computer room. Second floor is designated as memory care unit there is a combined activity and dining room along with a large patio. Large patio is accessible to all residents although currently memory care is not accessing the large patio due current isolation restrictions in place. Third floor consists of assisted living apartments. Fourth floor consists of assisted living apartments and gym. Fifth floor consists of assisted living apartments and game room, and sixth floor contains assisted living apartments and sky lounge. Each floor is accessible via two stairwells and two elevators. One stairwell is designated for memory care staff only and the other stairwell and two elevators are accessible to assisted living residents and staff. Elevators are cleaned three times a day at minimum. Food service is brought into memory care on a cart in disposable containers. Cart is then disinfected and removed from memory care and disinfected again before being brought down to kitchen.

No deficiencies were observed on this date. Exit interview conducted and a copy of this report sent via email.
