Investigation Findings: It was reported to the department that a resident was served with an eviction notice on June 22, 2026. The resident resides in independent living and stated that the facility offered a promotion providing two months of free rent for new residents and that because the resident has lived at the facility for several years, the resident believed the resident should have also received the promotional benefit and, as a result, did not pay rent for two months. R1 informed LPAs that since the facility has not offered R1 the promotion, R1 has not paid. R1 informed LPAs that R1 does not feel R1 is unlawfully being evicted. R1 understands that the facility is moving forward with an eviction due to R1 not paying and told LPAs, “I want to see what they are going to do.”. S1 informed LPAs that S1 served R1 with an eviction notice on 04/15/2026 and has been trying to work with R1 in finding assistance or legal aid to no avail. S1 informed LPAs that S1 has been trying to get R1 to seek assistance with R1 being rude and dismissive in response. LPAs reviewed the eviction notices sent to R1 and found the grounds for eviction to be based on nonpayment of rent for six (6) months. Based on interviews conducted and record review, the above allegation is UNSUBSTANTIATED.
Allegation: Staff verbally abused resident while in care
Investigation Findings: It was reported to the department that the facility manager told the resident that the resident would be homeless and they would "roll her out on the street" if evicted. R1 informed LPAs that S1 said this to R1 in private but later admitted it in front of S2 and W1. R1 said S1 was being rude and threaten that R1 would be rolled out of the facility. LPAs could not interview S2 as S2 was out of the community on leave. W1 informed LPAs that S1 was not being rude, S1 was just trying to impress on R1 what S1 had seen in the past and what R1 could expect to happen if R1 did not start paying rent. W1 stated that W1 did not hear S1 say “roll her out on the street”. W1 stated that S1 told R1 that the Sheriff would escort R1 out of the building. S1 informed LPAs that S1 was conveying to R1 the consequences of R1’s actions and what R1 could expect if the eviction did occur, including being escorted out of the community. Based on interviews conducted, the above allegation is UNSUBSTANTIATED.
Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.
No deficiencies were cited during this inspection.
