Allegation: Facility is in disrepair
The allegation is that the facility has an outdoor patio with loose tiles that shift when walked on, and that one of the facility’s automatic door opener buttons is non-operational. The tiles are outdoors and therefore affected by different air temperatures. When LPA toured the outdoor patio, LPA observed that none of the tiles were loose. S1 informed LPA that maintenance staff had addressed the issue, and secured the tiles so none of them rock when stepped on.
LPA tried every automatic door opener in the facility and found all of them in working condition. The narrative also states that the toilet flush levers are installed on the wrong side of the toilets. LPA observed that all the flush levers are on the left side of the toilet and in working condition. Based on observations the above allegation is UNSUBSTANTIATED.
Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove the alleged violations did or did not occur, therefore the allegations are UNSUBSTANTIATED.
No deficiencies were cited during this inspection.
