On 02/11/2026, at 2:30 PM, Licensing Program Analyst (LPA) James Sampair arrived unannounced to conduct this 10-day complaint visit. The LPA explained the nature of the visit with Executive Director (ED) Juan Ferrel.

LPA has determined through an interview with the ED and a review of the resident roster that the complainant does not live at this facility; therefore, the allegation is UNFOUNDED. A finding that is unfounded means the allegation is false, could not have happened, or is without a reasonable basis.

An exit interview was conducted and a copy of this report is provided.
