| LPA observed the following:
-No auditory signals on all exit doors.
-In room 2, there is not enough space for two beds
-No call button/ device for residents' use.
-No indicator for bedrooms
-The backyard consists of garden tools, storage that needs to be locked, and paint containers that need to be locked.
-Clear out the laundry spacing area.
- The widow screen is in disrepair
-No Ombudsman posters.
-The facility does not have a working landline phone.
LPA discussed obtaining $3M in liability insurance upon the granting of the license and the admission of the first resident and submitting a copy of the insurance certificate to LPA.
Prior to licensure, the following shall be corrected by June 8, 2026.
Issues were noted during inspection. LPA observed that the facility is not ready for licensure. This report will be submitted to the Central Applications Unit (CAU), and a final review of the application will be conducted. This facility is not yet licensed and is subject to final approval by CAU. Additional requirements may still be required.
An exit interview was conducted, and a copy of this report was provided.
