LPA Hiratsuka, conducted this complaint visit to deliver the findings.

LPA interviewed staff and resident in question. LPA also reviewed the medication log. Per the administrator the agency in charge of the resident's medications would verbally change medications and not follow-up in writing until a few days later. LPA was unable to make contact with the complainant. Resident stated they were given all medications and stated there was a communication issue between the agency and the facility. LPA cannot prove or disprove the allegation.

Resident stated they were left on the floor for hours. LPA was told the staff make rounds every two hours. LPA was told the resident has an issue with keeping track of time. The facility has cameras and it shows the staff making rounds. LPA cannot prove or disprove because each side has their own version of events.

LPA Hiratsuka conducted this uannounced complaint visit to deliver the findings of the complaint above.

LPA interviewed resident. Resident stated their medication is held and dispensed by the facility and that they have a lock box for personal hygiene items. Resident stated medication is despensed by the facility staff per prescription orders or as needed.

“This agency has investigated the complaint alleging; Facility is leaving medication accessible to resident. We have found that the complaint was unfounded, meaning that the allegation was false, could not have happened and/or is without a reasonable basis.

Due to the information gathered, LPA cannot determine the Facility is not dispensing medication per physician orders, and Resident fell and was not attended to in a timely manner. LPA finds allegation to be unsubstantiated. Although the allegations may have happened or are valid, there is not a preponderance of evidence to prove that the alleged violations occurred, and the findings are unsubstantiated.
