32 | On 11/13/24 and 11/18/24 LPA Laird conducted four parent interviews (P1-P4). All parents interviewed denied ever seeing children unattended. All parents interviewed stated they had always seen at least one staff with the children. All parents interviewed stated they had no concerns with the care and supervision being provided at the facility.
On 10/30/24, 11/18/24, and 11/19/24 LPA Laird conducted four staff interviews (S1-S4). All staff interviewed denied witnessing children unattended or unsupervised. All staff stated there is always at least one staff with the children. All staff interviewed stated they had never heard of concerns regarding staff supervision.
On 10/1/24 and 11/19/24 LPA Laird conducted an inspection of the facility and observed staff actively supervising children in care and operating within ratio requirements. LPA Laird observed a camera installed in a corner of the classroom and an alarm on the entrance doorway to the facility.
Based on statements made by the facility director that she temporarily goes into other rooms while alone at the facility, the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated. California Code of Regulations, (Title 22), is being cited on the attached LIC 9099D.
LPA Erica Laird informed Carla "Nikki" Small that this report dated 12/6/24 documents 1 Type A citation(s) which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
Also, LPA Erica Laird informed Carla "Nikki" Small to provide a copy of this licensing report dated 12/6/24 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
Exit interview conducted and report was reviewed with facility director/owner, Carla "Nikki" Small. Appeal rights were provided.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
