On 11/07/24 at 10:30am, LPA Elizabeth Friese conducted a case management inspection and met with Administrator Ann Nelson in response to an unusual incident report. A child (C1) had tripped and fallen on a play structure, hitting their leg. Subsequent x-rays were inconclusive. During the inspection the play area was toured, structure inspected and video of incident reviewed. LPA Friese determined that the equipment was age-appropriate, in good repair and that supervision was adequate. No deficiencies were cited.

Report reviewed with Administrator Ann Nelson and appeal rights provided.
Notice of site visit to be posted for 30 days.
