On 3/10/25 at 3:37pm, Licensing Program Analyst (LPA) Tammy Dutra conducted an unannounced complaint inspection, and met with facility representative, Chelsey Bleeke. It was alleged that there was a lack of supervision, specifically that the opening preschool teacher prepares food in the kitchen which is adjacent to the preschool room but does not have 100% supervision of children in care.

The Director corroborated the allegation stating that the opening preschool teacher does prepare food and may not have 100% supervision of preschool children when she is preparing the food.

Three staff members were interviewed on 1/6/25 and all staff corroborated that the opening preschool teacher does prepare breakfast in the kitchen while preschool children are in the room next to the kitchen. S1 said children are required to sit at the doorway while S3 prepares food. S2 indicated that children watch television while S3 prepares meals with the door open so they can periodically check on children in care.

S3 stated it was true they would prepare breakfast and did not have 100% supervision of children. S3 stated this lack of supervision has been addressed since the complaint was opened and now, they bring in another staff member to prepare meals so S3 can maintain 100% supervision of children in care.

Four parents were interviewed on 11/26/24 and 3/5/25 and two parents (P3,P4) indicated they have seen S3 preparing meals in the kitchen without a direct line of sight to children in care.

Based on the evidence obtained, the preponderance of evidence standard has been met, therefore the above allegation is found to be substantiated. California Code of Regulations, (Title 22), CCR 101229(a)(1) is being cited on the attached LIC 9099D.

Exit interview conducted and report was reviewed with the facility representative. Appeal rights were provided.

A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.

Facility has implemented scheduling another staff member in earlier to begin preparing meals for children in care. Staff understands they are required to have 100% visual supervision of all children in care. POC cleared on visit.

This was not met as evidenced by: based on interviews it was determined that children were left in the preschool room without 100% supervision while staff prepared meals. This poses a potential health, safety, or personal rights risk to persons in care.
