An unannounced case management inspection was conducted today at 11:37am by Licensing Program Analyst (LPA), Erica Laird. LPA met with facility director, Amanda Glenn in response to an Unusual Incident Report received by the Department on 2/12/25. Per the incident report, on 2/12/25 a child was taken to the playground by their outside specialist and left with another preschool program. The child was never left unattended and was returned to their correct classroom approximately 6 minutes after being left with the other program.

Facility director Amanda Glenn was interviewed on 2/13/25 regarding the incident. Amanda stated the facility took immediate action by notifying the outside specialist of the error and informed the child's guardian. Amanda stated the facility has implemented stricter sign in/sign out procedures for children who have outside services.

During today’s inspection, the facility was toured. LPA observed the sign in/sign out in the Child Pilot app and inspected the playground.

Based on interviews and observations, LPA Laird determined the facility was without fault.

There were no deficiencies cited during today’s inspection. Exit interview conducted and report was reviewed with director Amanda Glenn. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
