32 | On 1/31/25 LPA Laird conducted an interview with one staff (S1). S1 stated they had no knowledge of the time sheets. S1 stated licensee operates at close to full capacity every day but does not go over ratio.
On 4/9/25 LPA Laird conducted interviews with three parents (P1-P3). All parents interviewed denied licensee has asked them to falsify time sheets. Two parents interviewed stated there are times they forget to fill out their time sheets and they will fill them out at the end of the month.
On 1/31/25 LPA Laird took three photos of the sign in/ sign out sheets. On 2/18/25 LPA Laird obtained monthly time sheets from the Resource and Referral (R&R) agency. LPA Laird compared the time sheets to the times reflected on the R&R time sheets. LPA Laird determined the times reflected on both times sheets were similar, however, there were days where children were reflected on the R&R time sheets but not on the facility sign in/ sign out sheet. LPA Laird could not determine if this was an oversight or falsification.
Based on evidence obtained and interviews, there is not enough evidence at this time to determine whether or not the allegation occurred.
Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred, and the findings are unsubstantiated.
Exit interview conducted and report was reviewed with the licensee, Cristal Bolanos Gutierrez. Appeal rights were provided.
A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
