25 | On 12/06/2023 at 10:30am, an annual inspection was made to the facility by Licensing Program Analysts E. Laird, and E. Friese. At 11:00am the home was toured inside and outside. The licensee was supervising 7 children, and operating within the licensed capacity and ratio requirements. The facility’s operating hours are 6:00am 9:00pm, Monday–Friday. The floor plan submitted by the licensee was reviewed and verified. The off-limits areas of the home are the main house and were made inaccessible by doorknob cover. The children use the front yard as the outdoor play area and it is not fully fenced. Licensee understands 100% supervision is required whenever children are outside. There were no pools or other bodies of water observed in the yard.
5 children's records were reviewed at 10:41am. 1 staff record was reviewed at 10:50am. There are currently 2 adults living in the home.
The following deficiencies were cited: Infant observed with bottle in crib while napping. Flea/tick medicine observed in unlocked cabinet. Mandated reporter certificate is expired.
LPA's E. Laird and E. Friese informed licensee Wind Tucker that this report dated 12/06/2023 documents 2 Type A citatiions which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.
Also, LPA's E. Laird and E. Friese informed the licensee to provide a copy of this licensing report dated 12/06/2023 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.
