On 2/15/24 @ 8:40am Licensing Program Analyst (LPA) E. Laird and L. Friese conducted an unannounced case management inspection. An annual inspection was conducted on 12/06/23 which resulted in a citation for expired mandated reporter training for the licensee. Licensee was to submit proof of training to CCL by 1/06/24. CCL has not received licensee's mandated reporter certification as of the date of this inspection. LPA's asked licensee for mandated reporter certification. Licensee stated she could not access her certificate right now.

Upon inspection, LPA's observed licensee supervising 8 children, four of which were under 24 months. During inspection two more children showed up, one of which was under 24 months old. LPA's informed licensee she was acting out of ratio and could not accept more children in care until a qualified assistant arrived. Licensee had one adult in the home acting as an assistant. At 9:10am LPA's checked guardian and determined the adult assistant was not fingerprint cleared. LPA's informed licensee that the adult needed to leave the facility and not interact with the children. At 9:27am LPA's observed open chemicals under the bathroom sink which is accessible to the daycare children. LPA's took 1 photo of accessible chemicals. LPA's showed the chemicals to licensee. At 9:28am LPA's observed the door to the stairs was open. LPA's showed the accessible stairs to the licensee. LPA closed the door leading to the stairs.

Based on the observations, the following deficiencies were cited: accessible chemicals, uncleared adult, operating out of ratio, accessible stairs, and expired mandated reporter. A civil penalty is being assessed due to uncleared adult in the facility, 102370(d)(1) (see LIC 809D):

LPA's E. Laird and E. Friese informed licensee, Wind Tucker that this report dated 2/15/24 documents 2 Type A citation(s) which shall be posted for 30 consecutive days as there is an immediate risk to the health, safety, or personal rights of children in care.

Also, LPA E. Laird and E. Friese informed the licensee to provide a copy of this licensing report dated 2/15/24 that documents any Type A citation(s) to parents/guardians of all children currently enrolled by the next business day or the next day the children are in care, and to any newly enrolled parents/guardians for 12 months from the date of this report. A signed Acknowledgement of Receipt of Licensing Report (LIC 9224), or other written statement, must be placed in the child's file for verification.



A notice of site visit was given and must remain posted for 30 days.

Exit interview conducted and appeal rights were provided. This report was reviewed with the licensee, Wind Tucker.
