13 | On 9/29/22 at 11:40am Licensing Program Analyst (LPA) Emilia Grisak conducted an unannounced complaint inspection and met with Center Director Heather Carpenter.
It was alleged that facility is operating without a director, specifically that the assistant Director's last day was 7/22/22 and Director was out for a week afterwards. The Director was interviewed on 7/29/22 at 11:20am and denied the allegation. The Director stated that she was gone for a scheduled vacation on 7/12/22 - 7/19/22 and then was out sick and that she had an Assistant Director and teacher (S1) designated to be in charge while she was absent from the facility. Interviews were conducted with six staff, parents, and witnesses on 8/24/22, 9/6/22, and 9/27/22. The Assistant Director (AD) was interviewed on 9/6/22 and stated that duties were split between the AD and S1 while the Director was out of the facility. AD stated that after he left the facility, S1 was in charge while the Director was out of the facility. It was stated by three out of three staff that when the Director was out in July the Assistant Director and S1 were present and in charge. LPA observed a Designation of Facility Responsibility form was completed and submitted to CCL for Assistant Director on 11/1/21 and for S1 on 5/9/22. Although the allegation may have happened or is valid, there is not a preponderance of the evidence to prove that the alleged violation occurred, and the findings are unsubstantiated. |
