25 | An unannounced case management inspection was conducted today at 9:46am by Licensing Program Analyst (LPA), Erica Laird. LPA Laird met with facility owner/director Heather Carpenter in response to an Unusual Incident Report received by the Department on 8/26/24. Per the incident report, on 8/21/24 a staff (S1) was helping a child (C1) on the playground cross over some rocks. S1 tripped while holding C1 resulting in injury. S1 and C1 were transported via ambulance to the ER.
At 9:53am LPA Laird conducted an interview with facility owner/director Heather Carpenter. Heather stated S1 and C1 were okay and both are back at the facility. Heather stated the rocks are scheduled to be removed in two weeks.
S1 was interviewed on 9/17/24 and stated when the accident occurred, (C1) was standing near the rocks crying. S1 went to assist C1 and "slipped" on one of the rocks causing them to fall while holding C1. S1 stated they did not look down and thought they had stepped over the rocks but had accidentally stepped on the rocks. S1 stated C1 sustained a bump on the head and minor scratched. S1 stated they sustained minor injuries as well. S1 stated they were back at work within 2 days.
During today’s inspection, the playground was toured. LPA Laird took three (3) photos of the rocks.
There were no deficiencies cited during today’s inspection. Exit interview conducted and report was reviewed with facility director/owner Heather Carpenter. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100.
