25 | On 2/26/25 @ 8:36am an unannounced case management inspection was conducted by Licensing Program Analyst (LPA), Erica Laird. LPA met with Heather Carpenter, owner/director in response to an Unusual Incident Report received by the Department on 2/14/25. Per the incident report, on 2/10/25 a child (C1) was on the preschool playground when they fell off a play structure resulting in a fractured elbow.
LPA Laird conducted an interview with Heather Carpenter. Heather stated on 2/10/25 C1 was playing on one of the large tire structures and fell. Heather stated S1 went over to C1 and assessed the injury. Heather stated S1 notified her of the injury. Heather stated she looked at C1's arm and noticed it was swollen and had a bruise. Heather immediately called C1's parents who took C1 to the hospital where it was confirmed C1 had fractured their elbow.
On 2/26/25 LPA Laird conducted one staff (S1) interview. S1 stated C1 was playing on one of the large tire structures and fell. S1 went over to C1 to assess them. C1 was not complaining of pain at that time. S1 stated when they went inside and started to do art projects, C1 cried out in pain when turning their wrist. S1 stated they called director Heather Carpenter who immediately called C1's parents.
During today’s inspection, the facility was toured. LPA observed two large tire structures approximately 2 feet off the ground. LPA Laird took three photos.
There were no deficiencies cited during today’s inspection. Exit interview conducted and report was reviewed with owner/director Heather Carpenter. Appeal Rights were provided. A notice of site visit was given and must remain posted for 30 days. Failure to comply with posting requirements shall result in an immediate civil penalty of $100. |
