Application Specialist (AS) J.Snow with Dionna Lefkowitz for the purpose of an announced prelicensing change of ownership inspection on April 15, 2024. This is a change of ownership from 45404328 LITTLE DISCOVERIES PRESCHOOL – INFANT and 45401661 LITTLE DISCOVERIES DAYCARE & PRESCHOOL. Applicant requests a child care license to serve a total of 78 children, 42 of them preschool from age two to entry into kindergarten/first grade, 16 are infants age 0 to 24 months and 20 are toddlers age 18 to 36 months. The program will operate Monday through Friday from (hours of operation). Fire clearance was granted on 12/23/23 and will be reordered to match the room numbers and single license.
Applicant and Director acknowledge that the following documents must be posted at all times: License, Emergency Disaster Plan, Personal Rights, Parents' Rights Poster, car seat law, Effects of Lead Exposure brochure, menus, and daily schedule. LPA Snow discussed the forms that must be in each child's and each staff member's file. The facility will provide morning snack, lunch, and afternoon snack.
INDOOR ACTIVITY SPACE:
There are 2 preschool classrooms: Room #3 and #4. LPA Snow observed a sufficient amount of equipment, toys, tables, chairs, and cubbies. There is a first aid kit in each classroom and the office. LPA Snow observed cleaning disinfectants are appropriately stored and inaccessible to children. Applicant and Director stated medications will be stored in a lock box in the preschool classroom. Applicant stated there are no poisons or firearms on the premises. Applicant stated there will be labelled water bottles available for each child in care and they will refill at the sink. LPA Snow observed a functional carbon monoxide detector, plugged in the preschool classroom. Applicant stated the facility will use a digital sign-in/sign-out system. Report continues on 809-C.

Preschool: LPA, Snow measured the classroom #3 & #4. The total classroom space measures 1,473 of square feet, which accommodates the Applicant's requested number of preschool children. There is one preschool bathroom with 3 toilets, one urinal, and 4 sinks for the children. There's enough bathrooms, sinks and square footage to accommodate 42 preschool children.

Toddler: LPA, Snow measured the classroom #2, the total classroom space contains a total of 719.8 of square feet, which accommodates the Applicant's requested number of toddler children. There is one toddler bathroom with 3 toilets, one urinal, and 4 sinks for the children. There's enough bathrooms, sinks and square footage to accommodate 20 toddler children

Infant: LPA, Snow measured the classroom #1 the total classroom space contains a total of 775.04 of square feet, which accommodates the Applicant's request for #16 infants. They will use potty chairs to accommodate a maximum of 16 infants.
Individual measurements are recorded on the Capacity Worksheet (LIC 9024). Children who become ill during the day will be isolated in the office and will use the staff restroom, if necessary.

OUTDOOR ACTIVITY SPACE:
There are 3 separate play yards, one each for infant, toddler and preschoolers.
The play space is surrounded by a wood fence that is at least five feet tall. LPA, Snow observed a sufficient amount of equipment and toys. There are shaded areas supplied by an overhang and trees. Applicant and Director acknowledge staff must ensure children use age-appropriate equipment at all times. Applicant and Director stated the climbing structure is anchored into the ground. In the toddler yard the small tree house is rated age 2-12, the staff know that children must be at least 24 months to play on this climbing structure and will monitor. There is sufficient wood chips under climbing equipment to cushion a fall.

LPA, Snow measured the outdoor activity space. Individual measurements are recorded on the Capacity Worksheet (LIC 9024).
Preschool: The outdoor play area contains a total of 3,590 square feet, which accommodates the requested number of preschool children. The space allows for a maximum of 42 children.
Toddlers: the outdoor play area contains a total of 2074 square feet, which accommodates the applicant's requested number of toddlers. The space allows for a maximum of 20 children.
Infants: The outdoor play area contains a total of 1,218 square feet, which accommodates the Applicant's request for 16 infants. The space allows for a maximum of 16 infants.

The facility's Plan of Operation is located in the preschool file. Incidental Medical Services (IMS) policy was discussed. For IMS information see Evaluator Manual - Regulation Interpretations and Procedures for Child Care Centers Sections 101173 and 101226. When any IMS is provided, an updated Plan of Operation that includes IMS must be submitted to the Department.

LPA, Snow discussed the following: 100% supervision is required at all times, including in the bathroom; personal rights; criminal record clearances; inspection authority; reporting requirements; staff to children ratios and capacity; staff qualifications; and maintaining buildings and grounds.

AS discussed with Applicant any changes that may occur regarding the directors or an employee acting in the director's absence must be reported to department within 10 working days.

This facility evaluation report was reviewed and discussed with Applicant. Applicant was encouraged to the visit the Department's website at WWW.CDSS.CA.GOV for information regarding child care updates, forms, regulations and legislation pertaining to child care centers.

Conditions requiring correction prior to issuing a license:
